Desire to join J-CAM SSC & the National J-CAM for the year 2001 season.

I/we agree to:

1. Abide by the rules & regulations as set down in the J-CAM &

J-CAM SSC constitution & bylaws.

2. Abide by the rules of the J-CAM Quality Assurance (QA) Scheme.

3. Consent to the down grading of our fruit if the independent auditor finds our fruit is below the minimum QA grade standard for the grade as set down in the J-CAM QA scheme.

4. Sell all unused Jadefruit carton lids & fruit stickers back to J-CAM SSC if for any reason we terminate our membership or have our membership terminated.

5. Follow the Freshcare code of practice.

6. Have ready access to a fax machine & be readily contactable on it.

7. Financial member of ACAGA (Aust Custard Apple Growers Ass)

Enclosed is my/our annual marketing membership fee of $55.00 (includes GST), which includes both memberships to J-CAM & J-CAM SSC.
